Patients in West Palm Beach and along the Jupiter-to-Boca corridor ask me the same question almost every week: “Is your 4D Facelift the same as Fotona 4D?”

Short answer: no. Fotona 4D is a non-surgical laser treatment name. BellaNova’s 4D Facelift is a surgical plan that works from the inside out — skeleton, muscle, skin, then surface — for a natural result. Same two letters. Completely different medicine.

If you are comparing lasers, fillers, and surgery, that distinction matters before you spend money or time on the wrong path.

What Fotona 4D is

Fotona 4D is a branded laser protocol. It uses energy at the skin (and sometimes inside the mouth, depending on the package) to heat tissue, stimulate collagen, and create a temporary tightening or glow. There is no operating room, no incision to reposition deep facial layers, and no change to the facial bones.

For the right patient — mild laxity, early texture change, someone who wants little downtime — energy-based treatments can be useful. I use CO₂ resurfacing myself as a surface tool after surgery when it belongs in the plan. A laser can improve the canvas. It cannot rebuild the frame.

What BellaNova’s surgical 4D Facelift is

At BellaNova, 4D means four layers, in order:

  1. Skeleton — Facial bones lose projection with age (cheeks, chin, midface). Soft tissue drops when the support underneath shrinks. Sometimes the right answer includes implants or osteotomies. Pulling skin over a weak skeleton is why some facelifts look tight or odd.
  2. Muscle / fascia — Muscles migrate; the platysma separates in the neck; jowls form. That is not a skin-only problem. A deep plane release may be used here as a technical component when tissue is tethered — it is not the product name.
  3. Skin — Extra skin is removed and redraped without yanking it tight. Longevity comes from the deeper work, not from a drum-tight closure.
  4. Surface — After the lift is closed, CO₂ can refine tone and collagen when indicated.

Why Google (and patients) get confused

Search “4D facelift” without a city and you will see medspas advertising Fotona 4D next to surgeons talking about facelifts. The word 4D was used for marketing in both worlds. That is why our pages say, out loud: surgical 4D — not Fotona, not a liquid facelift.

If an ad promises a facelift result with “no downtime” and a laser, read carefully. You may be buying skin heating, not structural rejuvenation.

Who should not choose surgery yet

If your main issue is fine lines, pigment, or early softness, and your jawline still holds, we may talk about skin quality or a smaller plan first. I will tell you if surgery is the wrong tool. The consult exists to sort that out — not to pressure you into an operating room.

Who usually needs surgical 4D

  • Jowls and neck that fillers keep chasing
  • A prior lift that looks pulled because skin was tightened over weak support
  • Midface descent where bone projection was never ideal
  • Patients who want one coherent plan from a surgeon who can address bone and soft tissue (dual MD/DDS training)
