Brow lift or eyelid surgery: which addresses the heaviness?
A low brow can contribute to hooding above the eyes. Excess upper eyelid skin can also create heaviness, and some patients have both. During your examination, Dr. Carlotti assesses brow position with the forehead relaxed, eyelid skin and your natural asymmetry.
A brow lift changes brow position. Blepharoplasty addresses eyelid skin and, when appropriate, underlying fat. Some patients benefit from brow lift without eyelid surgery; others need both. The photographs below distinguish these procedures so you can compare cases accurately.
Am I a candidate for endoscopic brow lift?
You may benefit from a brow lift if you have:
- A brow that has descended relative to your natural facial proportions
- Heaviness at the outer third of the brow, giving a hooded or tired appearance
- Deep horizontal forehead lines from constant compensatory lifting
- Brow descent accompanied by forehead creases or frown lines
- A short distance between brow and upper lash line
Patients with a very high hairline may be better served by a hairline (pretrichial) approach, which lifts the brow without raising the hairline further. Your hairline, scalp, brow position and preferences help determine the approach. Your medical history, medications, nicotine use and ability to follow recovery instructions also affect candidacy.
How an endoscopic forehead lift is performed
Three to five short incisions are made behind the hairline. Through them, an endoscope provides direct visualization while the forehead soft tissue is released from the bone and the periosteum is elevated down to the orbital rims.
The corrugator and procerus muscles — the ones that create the vertical frown lines — can be selectively weakened under direct vision. The brow is then repositioned at the height determined preoperatively and fixed in place.
The amount and direction of lift are tailored to your anatomy. Planning considers the inner and outer brow, existing asymmetry and facial expression. The goal is a balanced position rather than excessive elevation.
Small incisions within hair-bearing scalp can help conceal scars. Scarring, hairline changes and thinning near incisions remain possible and are discussed before surgery.
Endoscopic brow lift recovery: what to expect
Plan for time away from work and social commitments. The timeline below is a general guide; combined eyelid surgery or laser treatment may change your recovery. You will receive instructions specific to your operation.

Risks and considerations
Risks include bleeding, infection, anesthesia complications, poor healing, visible scarring, hairline changes, hair thinning near incisions, numbness or itching, asymmetry, nerve injury affecting brow movement, and unsatisfactory brow position. Will I look permanently surprised?
The goal is a balanced brow position that suits your natural expression. Excessive elevation and asymmetry are possible risks. Dr. Carlotti plans the amount and direction of lift around your anatomy; no specific result can be guaranteed.
Does an endoscopic brow lift raise my hairline?
Hairline changes are possible. Dr. Carlotti assesses forehead height, hair density and incision placement before recommending an approach. A different technique may be appropriate for patients with a high hairline.
How long does a brow lift last?
A brow lift can provide lasting improvement, but aging continues. Longevity varies with anatomy, tissue quality and the procedure performed. There is no fixed number of years that applies to every patient.
Can I have a brow lift without blepharoplasty?

Are brow lift and forehead lift the same procedure?
The terms are often used interchangeably. Endoscopic describes an approach using a small camera through scalp incisions. The specific plan depends on your brow position, forehead and hairline.
Can neuromodulators replace a brow lift?
Neuromodulators may soften expression lines and provide a modest, temporary change in brow position in selected patients. They do not remove excess skin and are not equivalent to surgical tissue repositioning.
